Audit Application

Directory Information Removal or Prevention Disclosure
You can prevent your directory information from being disclosed to third parties by submitting this form. Visit our FERPA page for more information.

Education Record: Permission to Release
Give permission to the university to release education record information, other than a transcript, to a third party. Transcripts can be ordered separately here. This is a one-time release only. Any future release to the same party will require a new form.

Inspect and review your academic records with the Office of the Registrar.

Reference Letter Request
References include the following: recommendations for employment, for receipt of an honor or honorary recognition, for admission to an educational institution, for application for a scholarship or similar award, or any other pursuit of a student or former student in which a reference is necessary or desired. This form is required when a former student seeks a reference letter that contains non-directory information such as GPA, course grades, social security number, and information obtained from evaluations by others.

Submit this form if you need:

- A letter with specific information about your education at Seattle University.
- A duplicate diploma.

Submit this form if you need to add, update, or change the following information in your Seattle University records:
- Date of birth
- Preferred name
- Legal name
- Gender
- Ethnicity/Race
- Social Security Number
